When I receive this order form, I will review it and verify the cost of the item ordered and shipping charges when applicable. I will provide you with an estimated completion date as well as payment request through the e-mail address provided on the order form. Once the order and payment have been processed and completed, your item should be completed and shipped in approximately 2 weeks. An e-mail will also be sent to you once your item is shipped. Once received, please verify your order and contact me if there are any concerns.

If you are ordering multiple items, please indicate in the "number of" section to ensure that all of your order is processed together.
